I like to think Sam's body is full of scars. Let it be because of his numerous deaths and wounds or because of his self-experiments. But I also like it because it gives him a Frankenstein's Monster vibe, except he would be his own monster. "What if Frankenstein rebuilt his own body and became a monster instead of creating one" kind of vibe.

A lot of his scars are related to what happened in comics (for exemple he has one near the chest because of that one time he got stabbed by a spear, or his back because of when he was quadriplegic, or his heart etc etc...).

For his back, I made dragonfly shaped scars. To some, dragonflies are angels sent to Earth. And I like to think of Sam as a fallen angel (he has a very Lucifer-ious writing, especially in Immortal Hulk). And the reason this art was born is because I wanted to draw that lol, it's no deeper than that. I made a "shirt on" version as well because why not.
